Onderhoudsdosis: 20-40 mg/kg/dag

Vorm: tablet, granulaat, drank, siroop, IV oplossing

Indicaties: Alle aanvallen.

Contra-indicatie: mitochondriële stofwisselingsziektes, vrouwen in vruchtbare levensfase.

Dosisafhankelijke bijwerkingen: misselijkheid, braken, sedatie, tremor, Parkinsonisme, haaruitval, amenorroe, gewichtstoename

Ideosyncratische bijwerkingen:

hepatotoxiciteit, pancreatitis, trombocytopenie, encephalolopathie, polycysteus ovarium syndroom, teratogene effecten

Carbamazepine

(Tegretol)

Onderhoudsdosis: 10-20 mg/kg/dag

Vorm: tablet, retard tablet, suspensie, zetpil

Indicaties: partiële aanvallen, tonisch-clonische insulten.

Contra indicatie: Absence epilepsie, Aziatisch-Chinese afkomst, dan eerst HLA-B1502* bepalen

Dosisafhankelijke bijwerkingen: diplopie, ataxie, sedatie, misselijkheid, wazig zien, hoofdpijn, neutropenie, hyponatriemie

Ideosyncratische bijwerkingen: huiduitslag, Stevens-Johnson, agranulocytose, aplastische anemie, hepatotoxiciteit, lupus-achtig beeld, pseudolymfoom

Oxcarbazepine

(Trileptal)

Onderhoudsdosis: 15-30 mg/kg/dag

Vorm: tablet, suspensie

Indicaties: partiële aanvallen, tonisch-clonische insulten.

Contra indicatie: dosering aanpassen aan nierfunctie

Dosisafhankelijke bijwerkingen: duizeligheid, ataxie, vermoeidheid

Idiosyncratische bijwerkingen: huiduitslag, hyponatriëmie, pancreatitis, agranulocytose, aplastische anemie, hepatotoxiciteit, lupus-achtig beeld

Topiramaat

(Topamax)

Onderhoudsdosis: 1-5 mg/kg/d

Vorm: tabletten, sprinkle capsules

Indicaties: refractaire partiële aanvallen, GTC

Contra indicatie: geen, bij voorkeur niet combineren met ketogeen dieet

Dosisafhankelijke bijwerkingen:

Sedatie, ataxie, tintelingen, tremor,  woordvindingsstoornissen

Idiosyncratische bijwerkingen: eetlust vermindering, gewichtsverlies

Lamotrigine

(Lamictal)

Onderhoudsdosis: afhankelijk van andere gebruikte medicatie

Vorm: dispers tabletten

Indicaties: alle aanvallen

Contra-indicatie: orale anticonceptie

Dosisafhankelijke bijwerkingen:

misselijkheid, agitatie, hoofdpijn

Idiosyncratische bijwerkingen: huiduitslag, Stevens-Johnson syndroom, slapeloosheid. Bij rash direct afbouwen.

Levetiracetam

(Keppra)

Onderhoudsdosis: 15-50 mg/kg/dag

Vorm: Tablet, drank, iv oplossing

Indicaties: alle aanvallen

Contra-indicaties: dosis aanpassen bij nierfunctiestoornis.

Dosisafhankelijke bijwerkingen:

Sedatie, asthenie, ataxie, misselijkheid

Idiosyncratische bijwerkingen: rash, innerlijke boosheid, agressie

Fenobarbital

(Luminal

Onderhoudsdosis

Vorm: tabletten, drank in verschillende concentraties, iv oplossing

Indicaties: vooral neonatale convulsies

Contra-indicaties: let op verschillende drank voor sonde en orale toediening niet uitwisselbaar.

Dosisafhankelijke bijwerkingen:

Sedatie, duizeligheid, traagheid,

cognitieve dysfunctie

Idiosyncratische bijwerkingen: rash, Dupuytren, agitatie, tandvleeshyperplasie

Vigabatrin

(Sabril)

Onderhoudsdosis: 50-100 mg/kg/dag

Vorm: granulaat in sachets, tablet

Indicaties: syndroom van West

Contra-indicaties: myoclone epilepsie

Dosisafhankelijke bijwerkingen:

Sedatie, excitatie, hoofdpijn

Idiosyncratische bijwerkingen: irreversibele perifere retinadegeneratie

Lacosamide

Onderhoudsdosis: 100-200 mg/dag

Indicaties: partiele aanvallen

Contra-indicaties: onvoldoende ervaring bij kinderen < 16 jaar

Dosisafhankelijke bijwerkingen:

Duizeligheid, hoofdpijn, misselijkheid

Idiosyncratische bijwerkingen: rash

Zonisamide

(Zonegran)

Onderhoudsdosis:

Indicaties: refractaire partiële aanvallen, GTC

Contra-indicatie:

Slaperigheid, duizeligheid,

anorexie, agitatie, ataxie

Idiosyncratische bijwerkingen: Allergische huiduitslag, Stevens-Johnson syndroom

Felbamaat

(Taloxa)

Onderhoudsdosering: 7,5-15 mg/kg/d

Vorm: suspensie. Tabletten uit de handel.

Indicaties: atone aanvallen bij Lennox-Gastaut syndroom

Contra-indicatie: fructose intolerantie, bloeddyscrasie in voorgeschiedenis

Dosisafhankelijke bijwerkingen

Braken, gewichtsverlies

Idiosyncratische bijwerkingen:

Acuut leverfalen, aplastische anemie

Clobazam

(Frisium)

Onderhoudsdosering: 5-20 mg

Vorm: tabletten

Indicaties: therapie resistente aanvallen

Contra indicaties: gewenning; dosering langzaam

op- en afbouwen, slaapapnoe, leverinsufficientie

Dosisafhankelijke bijwerkingen:

Sedatie, spierslapte, moeheid, agitatie, slikstoornissen, gewichtstoename

Idiosyncratische bijwerkingen: huiduitslag

Ethosuximide

(Ethymal)

Onderhoudsdosis: 20-40 mg/kg/dag

Vorm: capsules, stroop

Indicaties: absences

Contra-indicaties: lever- of nierfunctiestoornissen

Dosisafhankelijke bijwerkingen:

Misselijkheid, buikpijn, vermoeidheid, hoofdpijn, hikken

Idiosyncratische bijwerkingen: SLE, psychotische reacties, aplastische anemie

Fenytoine

(Diphantoine)

Onderhoudsdosis: op basis van spiegel. Bij status epilepticus 20 mg/kg in 20 min oplaad

Vorm: tabletten, i.v. vloeistof, suspensie

Indicaties: status epilepticus

Contra indicatie: Niet lineaire kinetiek; doseren obv spiegel

Dosisafhankelijke bijwerkingen:

Nystagmus, ataxie, sedatie,

ritmestoornissen, dyskinesiën

Idiosyncratische bijwerkingen: huiduitslag, hepatotoxiciteit, tandvleeshyperplasie, vergroving gelaat, lupusachtig beeld, pseudolymfoom, Dupuytren, polyneuropathie, bloed dyscrasie. Bij parenteraal gebruik cardiale ritmestoornis.

Clonazepam

(Rivotril)

Onderhoudsdosis: leeftijdsafhankelijk Vorm: druppelvloeistof, tabletten, iv oplossing

Indicaties: status epilepticus, neonatale convulsies, myoclonieën, refractaire GTC

Contra indicaties: gewenning; dosering langzaam

op- en afbouwen, slaapapnoe, leverinsufficientie

Dosisafhankelijke bijwerkingen:

Sedatie, spierslapte, moeheid,

agitatie, hypersalivatie,

duizeligheid, hypotensie

Idiosyncratische bijwerkingen: pigmentatie, thrombocytopenie
